Please see 14.21 of the Chicago Manual of Style which sets out the basic structure of a bibliography entry. You can also find an example of a formatted bibliography in the second link below.
For undergraduate assignments, your bibliography can be single spaced with a blank line between entries. Manuscripts which are being submitted for publication should be double spaced to allow for copy editing.
